He was elected mayor of Burgas , the fourth largest city in Bulgaria, in 1995 as a candidate of the Bulgarian Socialist Party (BSP) . In 1999 he was confirmed in office with 61% of the vote. In 2003 he was re-elected as an independent candidate. With his term of office he is considered to be the “long-time ruling mayor of modern times” in Bulgaria.

On July 24, 2007, Joan Kostadinov was presented by the Union of Thracian Associations (an association for displaced persons) as a candidate for mayor's office. In August 2007, his former party, the BSP, also decided to support him as a candidate. In the local election on October 28, 2007, Kostadinov clearly failed in the first ballot. He was third with 16.53% behind Dimitar Nikolow ( GERB ) with 42.6% (supported by all democratic and conservative forces) and Valeri Simeonow with 22.84% (candidate for the nationalist coalition Ataka ). In the runoff election on November 4, 2007, Dimitar Nikolow finally won and replaced Joan Kostadonow.
