The Order of the Slovak National Uprising was donated by decision of the National Council No. 102 of August 23, 1945 in two classes and a commemorative medal and was intended as an award for all people who have made outstanding contributions to the defense, as well as themselves directly involved in the preparations and the defense and involved in the uprising. It could also be awarded posthumously.

The front of the commemorative medal made of bronze corresponds to the 1st and 2nd class. On the back there is the three-line inscription NA PAMÄT ZA ÚČASŤ NA POVSTÁNÍ (In memory of participation in the uprising). There is a linden leaf above and below the inscription.
