The orthopedic made-to-measure shoe differs from the "normal made-to-measure shoe " in that it is made by an orthopedic shoemaker . Each custom-made shoe is individually made, but only the orthopedic custom-made shoe has the task of alleviating defined health problems, compensating for restrictions, stabilizing them, correcting misalignments, normalizing the gait pattern, relieving pain or preventing deterioration. The prescription for a custom-made orthopedic shoe must be made by a doctor, stating the diagnosis and the necessary changes to the shoe. Often, however, a custom-made shoe is not necessary for foot changes; other means can be insoles , shoe adjustments on existing ready-made shoes or orthopedic ready-made shoes. B. for diabetes patients already have a soft bedding and seamless processing, but do not have to be made individually and are therefore significantly cheaper.

For the production, a last must first be made, which is done by means of an impression (e.g. plaster cast) as far as possible in the foot position to be achieved. The custom-made shoe is then made over this last, which is often shaft-high, but can also be made as a low shoe or as a boot - depending on the regulation. There are also special adjustments such as a soft insole, reinforcements, recesses or sole rolls.

Typical prescriptions for made-to-measure shoes are severe malpositions of the feet , paralysis, diabetes mellitus , open wounds or amputations in the foot area with partially preserved remaining feet.
