An acute cholinergic syndrome (also muscarinic syndrome ) is a pathological condition of the autonomic nervous system in which the vagus nerve ( parasympathetic nerve ) is overstimulated.
root cause
The syndrome usually occurs as a result of undesired drug effects or poisoning, but also after insect bites, see acetylcholine in animal and plant poisons .
It can occur as a temporary side effect when given irinotecan, which is used in chemotherapy .
Furthermore, pesticides and nerve agents such as sarin can lead to an acute cholinergic syndrome.
Clinical manifestations
The symptoms are:
- diarrhea
- sweat
- stomach pain
- watery eyes
- Visual disturbances with narrow pupils
- increased salivation
- chills
- Conjunctivitis
- Dilation of blood vessels with low blood pressure
- Drowsiness, malaise, dizziness
The frequency of occurrence in oncological treatment is given as 1%.

The treatment is performed by atropine , which, however, only a partial antidote ( antidote group), as atropine mainly binds to the muscarinic acetylcholine receptors, but not at the nicotinic acetylcholine receptors.
