American Eagle Flight 4184 was a flight operated by Simmons Airlines on October 31, 1994 .

The ATR 72-200 was en route from Indianapolis International Airport , Indiana , to Chicago O'Hare International Airport , Illinois . The aircraft got out of control and crashed due to icing of the wing trailing edges, where there were no heating mats for the de-icing system due to the design. All 68 inmates, including four crew members, were killed. It was the first ATR 72-200 crash.
