The granulomatous cheilitis is a rare disease with persistent swelling of the lips based on a granulomatous inflammation .
It is regarded as a subtype of orofacial granulomatosis .
Synonyms are: granulomatose orofaciale; OFG; Orofacial granulomatosis; granulomatous macrochyllium; Miescher cheilitis; english granulomatous cheilitis
The first description comes from 1945 by the Swiss dermatologist Alfred Guido Miescher (1887–1961).

The frequency is given as 0.08%.
root cause
The cause is not known.
The disease can occur idiopathically or as a skin manifestation in a granulomatous systemic disease such as Melkersson-Rosenthal syndrome , sarcoid or Crohn's disease (especially in children), but also in the context of food allergies .
Clinical manifestations
Clinical criteria are:
- Manifestation usually between the ages of 20 and 40, both sexes are affected equally often.
It is a disfiguring, initially recurring, then persistent granuloma-forming swelling of the lips.

The diagnosis is based on clinical findings.
The following must be distinguished from the differential diagnosis:
- Quincke's edema
- Herpes simplex
- Sarcoid
- Foreign body granulomas
