The single-nave church was built on the foundations of a Benedictine monastery church, which was consecrated in 1389. After this was abandoned due to the war, a new monastery church was built by the Franciscan brothers in 1476, dedicated to St. Francis of Assisi was consecrated. The church was also used as a burial place for the Cibo- Malaspina family , who owned the Duchy of Massa and Carrara . With the decisive support of his brother Cardinal Alderano Cibo, Duke Alberico II had the church rebuilt from 1660–71 by the architect Giovanni Francesco Bergamini and his son Alessandro Bergamini. A large marble high altar and two further altars in the transept were erected in the church, which was designed according to the Roman model. The painting of the Immaculate Conception is by Carlo Maratta , the painting of the Trinity by Luigi Garza has survived. Subsequently, Domenico Martinelli created a new chapel behind the right altar of the transept as a mausoleum of the ducal house. Polychrome marble of various origins was used here.

After Elisa Bonaparte , sister of Napoleon and Grand Duchess of Tuscany , demolished the nearby St. Peter's Church , its patronage was transferred to the St. Francis Church . With the creation of the diocese of Massa Carrara, the church was elevated to the Cathedral of St. Peter and Francis in 1823. In 1936 the marble facade was built according to a project by Cesario Fellini. In November 1964, Pope Paul VI bestowed . the cathedral the rank of minor basilica.
