On September 26, 1994, a Yakovlev Jak-40 crashed during a Cheremshanka Airlines flight from Krasnoyarsk to Tura , killing all 28 occupants.

The plane was a 20-year-old Yakovlev Yak-40 with the air vehicle registration RA-87468, the three engines of the type Ivchenko AI-25 was equipped.

The plane took off from Krasnoyarsk and headed for Tura. During the approach, the weather conditions deteriorated and the pilots were unable to land at the airport due to the fog . After the third aborted landing approach, the pilots decided to land at Wanawara Airport, which is 450 km south-southeast of Tura Airport. During the flight there, the aircraft ran out of fuel 41 km north-northwest of Wanawara at an altitude of 3,000 m. The three engines failed and the aircraft glided at a rate of descent of 7 m / s. Two helicopters and one Antonov An-24 were in the vicinity, the pilots of which suggested landing in the swamps . But the plane had already passed the swamps. The pilot attempted an emergency landing with retracted landing gear on the river Tschamba . The captain sent the other crew members into the cabin. His last words were: " Ищите нас на речке " ("Find us on the river"). Eventually , the plane hit some trees at a speed of 235 km / h, damaging the right wing , rolling the plane to the right and finally hitting the river bank of the Tschamba river at 17:38.

root cause

The investigations, which were hindered due to the lack of recordings of the conversations between the pilot and the air traffic controller, ultimately revealed the following factors as the cause of the accident:

  • incorrectly calculating the amount of fuel required for the flight to the alternative Wanawara airport
  • the sudden deterioration at Tura Airport, about which the pilots were not informed in time
  • the failure of Tura Airport staff to make observations when the weather deteriorated
  • the failure of the flight controller to properly review the work of the subordinate workers
  • the deviation during the first direct approach, which was probably caused by the short-term deactivation of the pre-entry signal
  • the failure of pilots to observe the parameters of the second and third approaches during weather conditions which were below the minimum
  • the choice of Wanawara Airport as the alternate airport instead of Baikit Airport , which was 110 km closer than Wanawara Airport
  • the choice of the wrong altitude and the early descent towards Wanawara.

During the inspection of the flight operations of Cheremshanka Airlines, deficiencies were found. The air traffic control workers at Tura airport had repeatedly drawn attention to the unacceptable level of work organization; however, no proper control was carried out.
