International Predictive Index

The International Prognostic Index (IPI) is a clinical scoring system in oncology that was developed in 1993 to assess the prognosis of patients with malignant non-Hodgkin lymphoma . The score is based on the retrospective analysis of the disease progression of 2031 non-Hodgkin lymphoma patients who were treated with doxorubicin- based chemotherapy (such as the CHOP regimen) between 1982 and 1987 . Current guidelines such as those guidelines Malignant lymphomas of DGHO differentiate the recommendations for chemotherapy and antibody therapy after INI score.

Scoring

One point is awarded for each of the following statements if they are true:

• Age> 60 years
• Stage III or IV disease spread of the Ann Arbor classification
• more than an extranodal stove
• poor general condition ( Karnofsky index ≤60% or ECOG score ≥2)
• increased LDH concentration in serum

Prognostic significance

The following risk groups can be derived from the point values ​​collected:

• low risk (0–1 points): 5-year survival rate 73%
• low intermediate risk (2 points): 5-year survival rate 51%
• high intermediate risk (3 points): 5-year survival rate 43%
• high risk (4–5 points) 5-year survival rate 26%

Age-adjusted International Predictive Index

For patients who have not yet reached the age of 60, an age- adapted international prognostic index is also used, which is only formed from the stage, general condition and the LDH value. This makes sense because the extranodal infestation does not appear to be an independent risk factor in younger sufferers. Here, a score of 1 corresponds to the low intermediate risk, a score of 2 to a high intermediate risk and a score of 3 to a high risk.
