Cerebellar bridge angle tumor

from Wikipedia, the free encyclopedia

A cerebellar bridge angle tumor is a tumor located in the angle between the cerebellum and the bridge , near the petrous bone .

etiology

Usually it is an acoustic neuroma , more rarely it can also be a meningioma , an epidermoid , a glomus jugular tumor , a cholesteatoma or a brain metastasis of another tumor.

Symptoms

According to the failures of the vestibulocochlear nerve , acoustic neuromas typically cause unilateral hearing disorders for months to years, often accompanied by tinnitus and dizziness . If the facial nerve is affected, the result is a disorder of the facial muscles. In meningiomas, the hearing impairment is less constant and there are more frequent disorders of the trigeminal nerve (with loss of sensitivity or pain) and facial nerve. In the further course also disorders of the cerebellum (can Adiadochokinese , hypotonia or deviation to the affected side) or pyramidal tract signs occur.

Cerebellar bridge symptoms

The combination of cranial nerve failure, cerebellar symptoms and cerebral pressure signs in processes in the area of the cerebellum or bridge is referred to as cerebellar bridge symptoms . In rare cases, it can also be based on inflammatory or vascular processes. An uncommon alternative name for these symptoms is Cushing's syndrome type II . In common parlance is called with Cushing syndrome to type I .

diagnosis

If, due to the symptoms mentioned above, there is a suspicion of a disease process in the area of the cerebellopontine angle, imaging tests of the brain should be carried out. A computed tomographic examination of the bony base of the skull typically reveals a radiographically recognizable widening of the internal acoustic pore . A tumor can be visualized by means of magnetic resonance imaging with a contrast medium. The measurement of acoustically evoked potentials can indicate a disorder of the auditory nerve.

therapy

The treatment of choice is surgical removal . Alternatively, radiosurgery can be used. Hydrocephalus or liquor fistulas threaten as complications .
