Mammillaria perezdelarosae grows solitary, spherical to elongated. The bright dark green body with the densely placed warts is up to 7.5 centimeters high and up to 4.5 centimeters in diameter. The warts do not contain milky juice . The axillae are sometimes provided with numerous bristles, otherwise woolly. 30 to 60 fine-needle radial spines, which are only 1 to 7 millimeters long, shine glassy white or glassy brown and cover the entire plant body in a comb shape. The 1 to 3  central spines are brown to black, red in the new shoot and 0.8 to 1.4 centimeters long. The longest and thickest are often also hooked.

The flowers appear as with all Mammillarias in wreath. They are whitish pink and reach a size and diameter of 2 centimeters. The fruits are whitish. The seeds are black to black-brown.
