After attending Grace Dieu Manor School and Ratcliffe College, Caruana completed a law degree at Queen Mary College in London and after graduating worked as a lawyer and partner at the law firm Triay & Triay . At the first election in the history of Gibraltar on May 16, 1991, he was for the Gibraltar Social Democrats (GSD) for the first time a member of the meeting house of Gibraltar (Gibraltar House of Assembly) elected. In the elections on January 16, 1992, Chief Minister Joe Bossano's Gibraltar Socialist Labor Party (GSLP) was again the strongest force with eight of the 15 seats ahead of the Gibraltar Social Democrats (GSD) under Peter Caruana, who had also been party chairman since May 1991. After the elections Caruana was new opposition leader (Leader of the Opposition) .

In the elections that followed on May 16, 1996, Bossano's GSLP suffered a defeat and only received seven seats, while Caruana's GSD now received eight seats. Thereupon Caruana became the new Chief Minister on May 17, 1996, while Bossano took over the function of opposition leader again. Also from the elections of February 10, 2000, November 27, 2003 and October 11, 2007, the Gibraltar Social Democrats emerged as the strongest parliamentary group in the House of Assembly with eight of the 15 and ten of the 17 seats from 2007 onwards February 2000, November 28, 2003 and October 13, 2007 was able to form further governments. The Gibraltar Socialist Labor Party, on the other hand, emerged as the second strongest force in these elections with seven of the 15 seats and 17 seats from 2007, so that Bossano remained Leader of the Opposition .

Under Fabian Picardo , the new chairman of the Gibraltar Socialist Labor Party and successor to Bossano, the GSLP won ten of the 17 seats in the Gibraltar parliament in the elections on December 8, 2011, after which Picardo succeeded Caruana as the new Chief Minister. Caruana, who was awarded the title of Crown Attorney (Queen's Counsel) in 1998, again took over the position of opposition leader and held this position until January 2013, when he was replaced by Daniel Feetham . On June 15, 2013, Caruana became Knight Commander of the Order of St. Michael and St. George (KCMG) and has had the addition of "Sir" since then. He decided not to run again in the November 26, 2015 elections and resigned from the Gibraltar Parliament after 24 years of membership.
