In the tropical sprue is one in the tropics occurring malabsorption syndrome of unknown origin. In addition to the residents of the endemic areas , people who visit or have visited these areas also become ill. Today it is assumed that the small intestine of these patients is chronically contaminated with enteropathogenic bacteria (which damage the gastrointestinal tract).

Symptoms and consequences

Abdominal complaints, flatulence, nausea, vomiting and diarrhea characterize the clinical picture. The insufficient intake of vitamin B 12 and folic acid often leads to the development of megaloblastic anemia . Fever is rare. The disease is progressive if left untreated .

diagnosis

The travel history is groundbreaking . Biopsies from the duodenum or jejunum can show signs of villus atrophy, but they can also show only minor changes.

therapy

The therapy of choice is to compensate for malabsorption by substituting folic acid and cobalamin and long-term antibiosis, e.g. B. with tetracycline .
