The forerunner of FSV Schmalkalden is SV 04 Schmalkalden , which was founded in 1904 . Until the end of the Second World War, SV 04 operated in the regional area of ​​the Gau West Thuringia. After SV 04 was dissolved in 1945, the club restarted in 1947 as SG Fortuna Schmalkalden . In the period that followed, the Thuringians climbed into the third-class district league Suhl after several promotions until 1954 .

By the mid-1950s, the sports community faced several name changes again. Initially renamed to ZBSG Schmalkalden and company sports association Feinprüf , in 1951 it was renamed to BSG Motor Mitte Schmalkalden . After a merger with Motor Ost , BSG appeared as Motor Schmalkalden from 1954 . On a sporting level, Motor Schmalkalden was on the spot until the late 1970s. After a temporary relegation to the Suhl district class, the general sporting conditions improved when the local tool combination went on board . In 1979 Motor Schmalkalden succeeded in advancing into the second-class GDR league with a narrow lead over Lok Meiningen . The BSG, now renamed Schmalkalden Tool Combine , developed into an elevator team over the next few years . The three climbs into the GDR league were immediately relegated.

After the reunification, the club was re-founded under its old name SV 04 . In terms of sport, the SV was integrated into the Thuringia regional league , from which the club quickly relegated to the district class in the next few years. In 1998, Schmalkalden managed to return to the national league for the next six seasons, with the football division spinning off from the previous club in 1999 as FSV Schmalkalden . After two relegations in 2004 and 2005, the FSV Schmalkalden played in the district league Thuringia (season 1) until the 2008/09 season. After first place in the table and winning the South Thuringia Championship, the FSV played in the regional class (West) Thuringia in 2009/10, from which, however, they were relegated immediately. In the 2017/18 season, the FSV played in the eight-class district Oberliga Rhön-Rennsteig. However, the men's team was canceled at the end of the season.
