On December 17, 2012, an Antonov An-26 of the Amazon Sky crashed into a steep mountain slope while crossing the Andes in the province of Yauyos at an altitude of 4578 m . The rescue teams only found the wreck a day later. None of the inmates survived.
course
The Antonov An-26 took off with four people and cargo on board at 10:08 a.m. local time from runway 15 in Lima in the direction of Aeródromo Las Malvinas, a private airfield operated by Petroplus , in the Cusco region . She continued the climb in extension of the orbit alignment and turned 30 km south of the airfield to the left to reach the waypoint ILPIP and a flight level of 21,000 feet . At 10:23 a.m., the crew requested the control tower to be allowed to switch from instrument flight to visual flight , at the height of 19,500 feet that had been reached.
At 10:27 a.m., the crew estimated the arrival in Malvinas at 11:27 a.m. About seven minutes later the aircraft approached the apex of the Andes, where a change in climate can often be observed. The last time there was radio contact with the aircraft was at 10:40, in which the arrival time was confirmed again. The further events in the next two minutes had to be reconstructed through the investigation.
Results on the course of the investigation
The flight data recorder and the voice recorder could be recovered from a tail section of the aircraft. The aircraft had slowed slightly (11 km / h) before 10:40, possibly an indication of the change in aerodynamics caused by ice. At 10:40 a.m., the copilot answered the radio traffic from Lima, instead of giving priority to the icing problem that has been known since that minute. According to the voice recording, the co-pilot reported first the failure of one engine and then the failure of both engines within a good 20 seconds at 10:41.
Cause of accident
The pilots had failed to properly handle the de-icing.
