Von Carnap was born the son of Peter von Carnap (1669-1736), mayor from 1722, and his wife Susanna Teschemacher (1670-1744). His brother was the mayor from 1739, Peter von Carnap . On his mother's side, he was the great-grandson of Johannes Teschemacher, who had been mayor in 1616. He also had numerous uncles and cousins who had also been mayors. On July 11, 1727, von Carnap married a distant relative, Helena Katharina von Carnap, a granddaughter of the multiple mayor Kaspar von Carnap (1648–1727). He had seven children with her, three of whom did not survive toddlerhood.

Johannes von Carnap worked as a merchant in Elberfeld and was proposed for the first and only time as mayor in 1737. He was able to prevail against the three competitors and thus became mayor that year. The following year he was city judge, otherwise he did not appear politically.
