Mammillaria muehlenpfordtii is a succulent, spherical plant with sunken vertices, which very quickly tends to divide dichotomously . Older plants often form large clusters of plants. The warts are gray-green and conical and covered with drooping bristles. The axillae are covered with numerous, fine white bristles that become more woolly in the flowering zone. At least 30–50 white to whitish yellow, 4 mm long radial spines are crowned with 2 to 6 upright central spines . The color of the central spines varies greatly between yellowish to brown and gray, and a length between 2 and 40 mm.
