Mandala Airlines Flight 91 was a domestic scheduled flights of Indonesian Mandala Airlines from airport Polonia to Soekarno-Hatta .

Flight history

On September 5, 2005 at 03:15 UTC + 7 (10:15 a.m. local time ), a Boeing 737-200 of the Indonesian low-cost airline Mandala Airlines with flight number 91 lost altitude shortly after taking off from Medan Airport and sped over the runway in a densely populated residential area. The plane went up in flames on a busy downtown street. As a result of the resulting explosions, panic broke out at the scene of the accident. At the time it was the worst accident in which a 737-200 was involved. The plane had previously belonged to Lufthansa (registration D-ABHK ) and then to Air Atlanta Iceland .

Victim

Of the 117 people on board, 5 crew members and 95 passengers were killed. 15 passengers were seriously injured, two more, a mother and her child, were uninjured. 49 people were killed and 26 seriously injured on the ground.

The only foreign passenger was the Malaye Ti Teow Chuan. He came, as did the governor of the province of North Sumatra , Rizal Nurdin and former governor of the province, Raja Inal Siregar killed. Few of the victims died from their injuries until after the crash.

The affected district was devastated by the crash, the fire and the subsequent explosions .

The aircraft was totally destroyed by the impact and the subsequent fire .

Survivors

Rohadi Sitepu , one of the surviving passengers, informed the private broadcaster Metro Television that all five other passengers in row 20 survived the crash. He reported that the Boeing lost altitude quickly after an explosion. On the ground, Rohadi jumped through a hole in the aircraft hull and ran away, hearing several more explosions behind him.

From the hospital, Freddy Ismail suspected the radio station El Shinta Radio that the aircraft had had technical difficulties after take-off: "After take-off, the noise of the engine became louder and the aircraft shook heavily before it fell".

Cause of crash

The flight recorders were recovered shortly after the accident and sent to the United States for investigation . According to the accident report, the pilots tried to take off without deployed buoyancy aids . It could not be clarified whether or why the Take Off warning , which should sound if the start configuration was incorrect, could not be heard. The wrong position of the buoyancy aids had remained undetected because the checklists had not been processed correctly.
