Mikulicz syndrome

The Mikulicz syndrome (after Johann Freiherr von Mikulicz-Radecki , 1892) describes a reactive swelling of the tear and salivary glands in various general and systemic diseases. The disease is also known as Dacryo-sialo-adenopathia atrophicans , Mikulicz-Radecki syndrome or Mikulicz-Sjogren's syndrome .

etiology

Mikulicz syndrome occurs with Hodgkin and non-Hodgkin lymphomas , chronic lymphocytic leukemia, and sarcoid . It is also occasionally observed in tuberculosis , syphilis , sialadenosis, and hyperthyroidism . The exact cause of Mikulicz syndrome is unknown, but it is believed to be an autoimmune disease .

Symptoms

Mikulicz's syndrome is characterized by the sudden onset of extreme dryness of the mouth (xerostomia), which is accompanied by difficulty swallowing and promotes the development of tooth decay , as the natural teeth cleaning by the saliva is missing. Further symptoms are enlarged lacrimal glands with reduced tear production, an enlargement of the parotid gland (glandula parotis), hard, painless swellings of the salivary glands . Symptoms may persist for a long time or may appear periodically and then go away. The symptoms of Mikulicz syndrome are very similar to Sjogren's syndrome . Some researchers suspect that they are identical disorders.

diagnosis

A biopsy of one of the swollen glands can be done to confirm the diagnosis . An ultrasound scan of the glands can help identify other reasons for the swelling of the glands.

Differential diagnosis

Therapy is carried out by treating the underlying disease. Otherwise, this disease is treated symptomatically . Artificial tears can be used to help maintain moisture in the eyes. Oral symptoms can be alleviated using artificial saliva.
