Secretory capacity of the thyroid

from Wikipedia, the free encyclopedia

The secretion capacity of the thyroid gland ( G T , also known as incretion capacity or, if calculated from hormone levels, referred to as SPINA-GT) defines the maximum stimulated amount of thyroxine that the thyroid can release into the bloodstream in one second .

detection

The secretion performance can be determined experimentally by measuring the T4 production of the thyroid gland after stimulation with a high TSH concentration (for example in the form of rhTSH , ie recombinant human TSH). In the past, secretion performance was also estimated by measuring the serum concentration of the protein-bound iodine isotope 131 I after administration of radioiodine . Due to the associated high radiation exposure , this procedure is no longer used in routine examinations.

In vivo , SPINA-GT can also with

or

can be calculated from serum levels of TSH and free or total thyroxine . This physiologically based equation is the most widely used standard method for determining the secretion capacity of the thyroid gland today.

Constant parameters of the equations are:

: Dilution factor for T4 (reciprocal of the apparent volume of distribution , 0.1 l −1 ) : clearance exponent for T4 (1.1e-6 sec −1 ) K 41 : dissociation constant T4- TBG (2e10 l / mol) K 42 : dissociation constant T4 - TBPA (2e8 l / mol) D T : EC 50 for TSH (2.75 mU / l)

The quotient of SPINA-GT and thyroid volume determined sonographically is called the specific secretion capacity (SPINA-GTs).

The equations and their parameters are calibrated for adults with a body mass of 70 kg or a plasma volume of approx. 2.5 l.

Clinical significance

SPINA-GT is increased in the case of primary hyperthyroidism and reduced in the case of primary hypothyroidism . The secretion performance correlates with the sonographically determined thyroid volume and showed in a longitudinal study a higher reliability than TSH , FT4 or FT3 . A negative correlation between GT and creatinine clearance was observed in one study . In the initial phase of a non-thyroidal illness syndrome , SPINA-GT passager can be slightly elevated. On the other hand, SPINA-GT may be reduced in certain chronic diseases such as chronic fatigue syndrome .

In men with hypothyroidism , both SPINA-GT and SPINA-GD correlate negatively with erectile function and sexual satisfaction. In women with thyrotoxicosis , increased secretion rates predict sexual dysfunction and a higher degree of depression .

In people with autoimmune thyroid disease and a borderline low vitamin D supply, the secretion performance increases under therapy with 25-hydroxycholecalciferol . Therapy with selenomethionine seems to increase this effect. An adequate supply of vitamin D is also necessary for the effect that statins have on secretion performance.

The specific secretion capacity (SPINA-GTs) is increased in obesity and autoimmune thyroiditis .
