The Cystic adenomatoid malformation of the lung (CCAM abbreviation of English congenital cystic adenomatoid malformation ) is a lung malformation that to the group of hamartomas belongs.

Cystic adenomatoid malformation in a newborn on chest x-ray. You can see the large cystic changes in the left lung, which, due to their space-occupying effect, have led to a mediastinal shift to the right.

Types according to Stocker:

  • Type I: (50%) One or more cysts with a diameter of 2–10 cm , in some cases also smaller cysts. Some of the cysts contain mucus.
  • Type II: (40%) multiple small, uniform cysts 0.5–2 cm in diameter.
  • Type III: (<10%) microscopic cysts. The imaging shows a solid mass.

Symptoms

Infections of the bronchi and lungs , shortness of breath , respiratory failure , heart failure and failure to thrive.

There is an association with pleuro-pulmonary blastoma .

Diagnosis

The CCAM can be seen prenatally on ultrasound . A close check after the diagnosis is made is necessary, as it can lead to rapid growth. However, 56% of the CCAMs discovered are regressing.

After the birth: X-ray of the lungs, CT , possibly bronchoscopy , NMR

therapy

If the death of the child is feared due to a strong increase in size before birth, an open prenatal removal of the process can be carried out in an appropriate center.

If the CCAM causes symptoms, surgical removal is recommended; asymptomatic cases are still controversial.
