Air Tahiti flight 805 was a domestic scheduled flight of Air Tahiti from Hiva Oa to Nuku Hiva , on which a Dornier Do 228-212 crashed on April 18, 1991 after an engine failure and a resulting pilot error. Ten people lost their lives in the impact.
Flight history
The Dornier 228 took off at 11:10 a.m. local time (10:40 p.m. CET) in Hiva Oa with 22 people on board. At 11:43 a.m. local time (11:13 p.m. CET), the first contact was made with air traffic control in Nuku Hiva, which informed the crew that the landing should take place on runway 6. At 11:56 a.m. local time (11:26 p.m. CET), air traffic control heard one of the pilots say that he would be approaching runway 24. Air traffic control denied this and repeated the request to land on runway 6. At this point in time, the aircraft was only at an altitude of approximately 700 ft (approx. 210 m ) and 400 meters from runway 24, and both propellers failed. As a result, the aircraft made left and right turns before it hit the water behind the runway. The impact killed ten people.
examination
According to the investigation, the crash occurred as a result of the pilots' inadequate response to an engine failure. This alone should not have caused the plane to crash. Furthermore, the final report records a probable drunkenness of the crew, a lack of rigor in obtaining the type qualification for a Dornier and deficiencies in the technical control of the aircraft by Air Tahiti as contributing factors.

The 38-year-old male captain received his commercial pilot license on May 15, 1979. It was valid until February 29, 1992 at the time of the crash. His approval was initially for a Beechcraft Baron . Before the crash, he obtained additional licenses for the Fokker F-27 , de Havilland Canada DHC-6 , Britten-Norman BN-2 Islander , ATR 42 and Dornier Do 228 . The latter on December 3, 1990. Since August 6, 1979 he worked for Air Tahiti, since November 1980 as a captain. The 38-year-old had 8,115 flight hours, 112 of them with the Dornier.
The 42-year-old male co-pilot received his commercial pilot's license on July 21, 1983. This was valid from April 1991 to July 31, 1991. It was first approved for a Piper Apache . In the following years he obtained licenses for Britten-Norman BN-2 Islander, de Havilland Canada DHC-6, ATR 42 and Dornier Do 228. The latter on December 8, 1990. Having previously worked for Air Moorea , he has worked since on May 25, 1989 for Air Tahiti. He had a flight experience of 4,766 flight hours, 95 of which with the Dornier.
