Sudden turbo-compressor rotation was noted twice while starting engine No. 2 (center) . root cause
The cause of the accident was the bursting of the air starter while climbing , which damaged fuel lines and caused a fire, which then damaged control surfaces and hydraulic lines and made the aircraft uncontrollable. The cockpit crew should not have taxied to take off, so they underestimated the seriousness of the warning. In the operating manuals , however, there were no procedures to be followed for this incident, and the cockpit displays did not provide any further information on the condition of the air starter.
