Candida parapsilosis ( synonym : Candida parakrusei ) is an asexual diploid pathogenic yeast that has become an important factor in sepsis and wound and tissueinfectionsin immunocompromised patients andcausesabout 15% of Candida infections. Strains of Candida parapsilosis have historically been classified into Group I, II, or III on the basis of molecular fingerprint. The immune system is an important factor in Candida parapsilosis infections. In contrast to Candida albicans and Candida tropicalis , Candida parapsilosis is not an obligatory human pathogen, but has also been isolated from non-human sources such as domestic animals, insects and soil. Candida parapsilosis is also a normal human commensal and it is one of the most isolated fungi from human hands. Candida parapsilosis tends to form biofilms and is then more difficult to treat.
There are various risk factors that support the fungus in colonizing the human body. Immunosuppressed individuals with AIDS / HIV, oncological patients, newborns, operated patients and elderly patients, especially after intestinal operations and intestinal diseases, have a high risk of infection with Candida parapsilosis . As early as 1968, Blank u. a. Carcinogenic properties found in Candida parapsilosis in mice. With systemic infection, Candida parapsilosis can infect the central nervous system as well as the muscles.
Well-known clinical pictures are endocarditis after cardiac catheters or cardiovascular interventions, peritonitis after peritoneal dialysis , endophthalmitis after lens implantation, septic arthritis, nosocomial infection , pneumonia (rarely), nail, corneal and skin mycoses, infestation of the gastrointestinal tract.
Culture
Candida parapsilosis grows on Sabouraud agar as white to beige colonies that are smooth and shiny to slightly wrinkled. On RAT, pseudomycelium is formed from chains of long cells with clusters of blastospores attached.
genetics
The molecules of mitochondrial DNA (mtDNA) are packaged in nucleic acid-protein complexes, also known as mitochondrial nucleoids (mt nucleoids). Investigations of the band intensities after DNA staining led to an estimate that C. parapsilosis contains 14 chromosomes (7 pairs) with a total genome size of around 26.3 Mb (Doi et al. 1992).
treatment
There is currently no consistent medical opinion on the treatment of Candida parapsilosis . Historically, amphotericin B was used. Miconazole , nystatin , terbinafine and triazoles such as fluconazole , voriconazole and posaconazole are modern alternatives.
