Case conference

A case conference is a session in which the future course of action regarding a patient or client is discussed and decided. As far as possible, all those involved in the helper system should take part in this meeting and - depending on the problem or their own ability to participate - also the person concerned. Most of the time, case conferences are interdisciplinary and multi- professional; all professions dealing with the patient take part equally.

Case conferences are a classic instrument of mutual information and coordination of assistance in health care and nursing , home care , life counseling , medicine , education , psychotherapy , crime prevention and social work . The case manager is either the patient's legal representative or a confidant or the person from the group of helpers who is most intensely familiar with the case, or the professional with the highest specialist knowledge. Case conferences are indispensable in the case of serious changes in the life of the person affected - retraining , relocation , deterioration or improvement of the state of health , dissatisfaction with care - and useful and appropriate in the interface management between inpatient and home care.

The goals of a case conference are to understand the case, define goals, roles and responsibilities, and document the agreements reached.

The case supervision must be clearly distinguished from the case conference , in which the same group of people can participate (with the exception of the patient himself), but in which the problems and burdens of the helpers are to be reflected on and reduced. The case conference, on the other hand, is used to determine procedures for the benefit of the patient, the distribution of tasks, as well as getting to know the group of helpers and the coordination and goal orientation of the participants.
