Flight history

The plane took off from Mataram at around 3:00 p.m. and rose to a cruising altitude of 31,000 feet. During the beginning of the descent, the master decided to deviate from the planned route due to thunderstorms. At 4:19 p.m., the machine first reached the area of ​​a strong thunderstorm cell with turbulence, strong thunderstorms and extreme precipitation and hail. 90 seconds later, when the aircraft was just at about 18,000 feet, both engines suffered a stall , causing the Boeing to lose both its thrust and power. The pilots tried several times to restart the engines, but they did not succeed. They then tried to restore the machine's electrical power supply by starting the auxiliary power unit . This attempt also failed, which meant that an emergency call could not be made to flight control. The pilots then decided to make an emergency dive on the Solo River, which had come into view under the cloud layer. The rear hull first touched down on the water at 1629 and remained intact after the splash. The fuselage and wings as well as the main control instruments remained intact. Nevertheless, the machine later had to be written off as a total write-off. Since the nose was raised relatively high when landing, the stern hit the first thing, which tore away the area of ​​the cabin floor where two flight attendants were sitting. One of the two was killed. Several other inmates were injured.

Rescue operation

There were only two doors available for evacuation. Residents of a nearby village helped the passengers with the rescue and looked after them. After the evacuation, the pilots called the tower by mobile phone, reported about the emergency landing and reported the location. Rescue workers were then sent to the site of the accident, which they reached about two hours after the ditching.

The investigation was led by the Indonesian Transport Safety Authority , NTSC . Their final report reveals that the main cause of the accident was that the pilot training in interpreting weather radar data was informal and inadequate. In addition, it could be that the precipitation was so dense that a possible route through the storm could be identified on radar data, as this could have attenuated the radar reflections. The cause of the flame stall in both engines was clearly determined to be that an extreme hailstorm, which also damaged the front of the aircraft, together with increased rainfall, was beyond the tolerance of the engines.

Flight number 421 from Garuda Indonesia is now used for one of the multi-day flight connections between the Garuda bases in Denpasar (Bali) and Jakarta-Soekarno-Hatta .
