The Hyperthecosis ovarii denotes a familial occurring closely with PCOS related disease with hyperplasia of theca cells with the main features of virilization , hirsutism and infertility .

Synonyms are: hyperthecosis; Thecomatosis

The name was first used in 1971.

pathology

Histologically, there is hyperplasia of the theca cells in atretic ovarian follicles , an overall decreased number of follicles and an increase in stromal tissue . The plasma levels of dihydrotestosterone , testosterone and luteinizing hormone (LH) are increased, those of estradiol and follicle-stimulating hormone (FSH) are decreased.

Some male family members have been found to have low testosterone and elevated LH and FSH levels.

A presumably autosomal dominant inheritance is assumed.

In contrast to PCOS, there are no polycystic changes.

Clinical manifestations

Clinical criteria are:

In addition, there is often acanthosis nigricans .

diagnosis

In addition to the findings of the clinical examination , the results of laboratory medicine are decisive; medical imaging can document the enlargement of the ovaries, which is usually present on both sides, and is also of limited value.

Differential diagnosis

A distinction must be made between PCOS and a fibroma of the ovary.
