José Sanfilippo began his career with CA San Lorenzo de Almagro in his hometown of Buenos Aires . In the service of this team, he scored 200 goals in 260 games. He was the top scorer in the Argentine Primera División in 1958, 1959, 1960 and 1961 . In 1960 he even scored the most goals in all South American first leagues. After nine years at San Lorenzo, he went on to Boca Juniors , the club from the poor La Boca district , which had already produced numerous Argentine top stars, including Diego Maradona . But Sanfilippo only lasted a year in La Boca.

The contract was terminated due to disciplinary problems. In May 1964 he joined Nacional Montevideo and made his debut with the "Bolsos" as part of the European tour. In the Estadio Centenario he ran for the first time as part of a friendly game Nacionals against Club Atlético Colón . He impressed by scoring four of the five goals of the Uruguayan team. In the following years he played a total of 17 games and scored 16 times in the opposing goal. A few weeks after his debut, he was seriously injured in a friendly against Brazilian club Vasco da Gama on July 25th . He suffered a broken tibia and fibula. On October 17, 1965, he celebrated a brilliant comeback in the 3-0 victory against Danubio as a three-time goalscorer. During the Uruguayan championship that year, he scored eleven times in eleven appearances. By the end of his engagement in 1966, his personal record with the Montevideans showed 29 goals in 32 games across all competitions, including friendly matches and the European tour.

Then he returned home to CA Banfield . But he was not happy in Banfield either and went abroad again to Bangu Atlético Clube in Brazil . After staying there for a year, he moved within Brazil to EC Bahia , where he stayed for three years. In Bahia he was again top scorer in a league when he scored 46 goals in the 1969 season in the Torneio Roberto Gomes Pedrosa , the predecessor of today's Brazilian championship. Then he went back to his hometown club CA San Lorenzo de Almagro for a year and ended his career in 1972 at the age of 35. He had previously won the championship title with San Lorenzo. In 1978, he was persuaded to return to professional football at the age of 43 and played another season at Club Atlético San Miguel before finally ending his career.

José Sanfilippo played 29 times for the national team of Argentina between 1957 and 1962. He scored 21 goals in these 29 international matches. His first major tournament was the World Cup in Sweden in 1958. There he was not used and Argentina failed in the preliminary round. Four years later in Chile , Argentina and José Sanfilippo again finished after the preliminary round.
