Lichen planus moniliformis

The Lichen ruber moniliformis is a rare chronic inflammatory skin disease with a strong itchy papules , hyperkeratosis and skin defects ( excoriation ).

Synonyms are: lichenoid trikeratosis; Kaposi-Bureau-Barrière-Grupper Syndrome; Latin keratosis lichenoides chronica; Lichen verrucosus et reticularis; Porokeratosis striata ; English Wise-Rein disease

The first description and name was given in 1886 by Moriz Kaposi .

The other names refer to descriptions from 1936 by the American dermatologist Fred Wise together with Charles R. Rein.

Spread and cause

The disease occurs more often around the age of 20 and then again around the age of 50, but can already occur in children. Men get sick more often than women.

The cause is not known. It could be a variant of  lichen planus or  hyperkeratosis follicularis et parafollicularis in cutem penetrans (Kyrle's disease).

Clinical manifestations

Clinical criteria are:

  • Hyperkeratotic, red-brown, strongly itchy papules confluent over a large area
  • Spread from one localization, mostly to the trunk and extensor sides of the extremities
  • Facial involvement as in seborrheic eczema  or rosacea
  • possible involvement of the oral mucosa, larynx and eyes, as well as changes in the nails
