MOBILIS (= M ultizentrisch o rganisierte b ewegungsorientierte I NITIATIVE the L layer style change i n S elbstverantwortung; lat. : Mobilis = "movable") is an interdisciplinary training program for the treatment of obesity in adults ( BMI 30-40 kg / m²).

history

The initiator was in 2002 from the University Medical Center Freiburg , Department of Rehabilitation and Preventive Sports Medicine (Aloys Berg) and the German Sport University Cologne , Institute for Cardiovascular Research and Sports Medicine (Hans-Georg Predel). After a successful model phase with the first groups in Freiburg im Breisgau and Ratingen in 2004, MOBILIS has been offered at various German locations since 2005.

By 2007, the program was on the club organizes AG prevention and heart groups eV. Since the foundation of the MOBILIS eV (March 5, 2007) it was responsible for the implementation and coordination of the program until 2017/18. concept

The aim of the program is to enable the participants to lead a health-conscious lifestyle in self-responsibility . Under professional guidance, the participants should be able to shape their exercise and eating behavior in such a way that the desired health effects ( weight reduction and improvement of risk factors such as arterial hypertension , type 2 diabetes mellitus or lipid metabolism disorders ) can be achieved in the long term.

It encompasses the four main therapeutic areas of exercise and sport, psychology and education, nutrition, and medicine and health. The focus of the concept is physical activity with 40 practical exercise units (endurance, strength, coordination and relaxation training as well as small games).

In 20 theoretical group sessions it is primarily about the volitional implementation of the participants' intentions. Central interventions are used. This includes self-observation training, conscious use of implementation intentions , anticipation of internal and external obstacles (barriers) and the development of suitable counter-strategies (barrier management).

It is implemented uniformly nationwide with the help of standardized teaching materials by trained teams of trainers and doctors (sports teachers / sports scientists, psychologists / pedagogues, certified nutritionists, doctors).

No weight loss products, formula diets or drugs for weight loss are used as part of the program .

With its concept, it meets the requirements of a quality-assured outpatient obesity program as specified by the S3 guidelines on obesity and the criteria of the specialist societies DAG , DDG , DGE , DGEM and DAEM .

Program structure

It is a closed, structured group program with a one-year term. The following intervention phases are specified as the program sequence:

  • Phase I (8 weeks): initial medical examination, 8 exercise units, 8 group sessions (topics: 1 × prelude, 1 × theory of exercise, 3 × nutrition and 3 × psychology)
  • Phase II (16–18 weeks): 22 exercise units, 4 group sessions (topics: 4 × psychology), 1 nutritional success check, 1 practice session on nutrition, medical interim examination
  • Phase III (6 months): 10 exercise units, 6 group sessions (topics: 5 × psychology), 1 practice session on nutrition, final medical examination

All participants receive a detailed final report with all examination results for their family doctor.

Participation requirements

Applicants must have reached the age of 18, be more than 20% overweight ( BMI ≥ 30 kg / m²), be physically able to participate in an exercise program and undergo an initial medical examination.

People with an overweight of more than 60% ( BMI > 40 kg / m²), people who suffer from type 1 diabetes mellitus , people with an eating disorder ( bulimia , bulimia nervosa , binge eating disorder ) and people are excluded from participation who are taking psychotropic drugs , weight loss medications and / or appetite suppressants or who have undergone gastric surgery. Another exclusion criterion is a malignant tumor disease with a disease-free state for less than 5 years.

The program is only suitable for people who are willing to question old patterns of behavior and work independently on a new lifestyle. This requires a high level of personal commitment and takes a lot of time. The closed group structure often results in longer waiting times before the program starts.

costs

The costs for the entire program were 785.00 euros per participant until December 31, 2017. Based on a framework agreement with BARMER GEK , flat-rate reimbursement of costs in accordance with Section 43 (1) No. 2 SGB ​​V in the amount of EUR 685.00 (personal contribution: EUR 100.00) was possible from 2004 to 2017 . Other cash registers, such as B. DAK-Gesundheit also participated. Since January 1, 2018, the question of reimbursement for each location has to be clarified locally with the local health insurance companies.

Evaluation

The scientific evaluation of the course of the project and the effect of the intervention was carried out by the University Clinic Freiburg , Medical University Clinic, Dept. Rehabilitative and Preventive Sports Medicine from January 2004 to June 2011. The data was collected by MOBILIS eV from July 2011 to the end of 2018. Results were regularly published and discussed in specialist journals. Two dissertations at the University of Freiburg from 2011 and 2013 also deal with the effectiveness of the concept, as well as a long-term study by the University of Konstanz with the cost-benefit ratio of the program .
