The retired police consultant Frank Black, formerly of the FBI worked, has returned with his wife and daughter in his hometown. He offers his assistance in a case in which he is supported by the mysterious organization Millennium Group. Frank is supposed to use his ability to see through the killer's eyes to catch the killer. However, the current case pulls him back into a world from which he was actually trying to escape.

Frank travels to Portland , Oregon to investigate the murder of an animal rights activist. He meets with Jim Horn, a profiler and candidate for the Millennium Group. Horn is troubled by the imminent divorce and there is tension between him and Frank. When the killer strikes again, Horn loses control, and Frank not only has to catch the killer, who leaves gruesome news on his victims, but also has to fight Horn, who in his own way wants to settle accounts with the killer.

Frank is supposed to help find a murderer who sent his severed tongue to his victim's wife. He believes the murders are linked to other unsolved serial killings that also involved sending body parts through the mail.
