SOFA score

from Wikipedia, the free encyclopedia

SOFA-Score is the abbreviation for Sepsis-related organ failure assessment score (about "measure for the assessment of organ failure in sepsis "), sometimes also Sequential organ failure assessment score ("measure for repeated assessment of organ failure"). This score is used to describe the condition of a patient with sepsis and the extent of organ damage during treatment in the intensive care unit . The score consists of six different measures of the condition of the respiratory function , the cardiovascular system , liver function, clotting , the renal and neurological status. Both the mean of the individual scores and the highest value have a predictive power for the patient's outcome.

calculation

Organ system parameter 1 2 3 4th
breathing PaO2 / FiO2 <400 mmHg <300 mmHg <200 mmHg and artificial ventilation <100 mmHg and artificial ventilation
Nervous system Glasgow Coma Scale (GCS) 13-14 10-12 6-9 <6
Cardiovascular system (Dosages in µg / kg / min) MAP <70 mm / Hg Dopamine ≤ 5 or
dobutamine (any dose)
Dopamine> 5 or
adrenaline ≤ 0.1 or
noradrenaline ≤ 0.1
Dopamine> 15 or
adrenaline> 0.1 or
noradrenaline> 0.1
liver Bilirubin 1.2–1.9 mg / dl
[20–32 μmol / l]
2.0–5.9 mg / dl
[33–101 μmol / l]
6.0–11.9 mg / dl
[102–204 μmol / l]
> 12.0 mg / dl
[> 204 μmol / l]
Coagulation Platelets <150,000 / µl <100,000 / µl <50,000 / µl <20,000 / µl
kidney Creatinine 1.2–1.9 mg / dl
[110–170 μmol / l]
2.0–3.4 mg / dl
[171–299 μmol / l]
3.5–4.9 mg / dl
[300–440 μmol / l]
(or urine <500 ml / d)
> 5.0 mg / dl
[> 440 μmol / l]
(or urine <200 ml / d)

If an organ system is not impaired according to the specified criteria, 0 points are calculated for this category.

qSOFA

The qSOFA (= quickSOFA) score was introduced in the new definition of Sepsis 2016 for the detection of high-risk patients due to sepsis outside of intensive care units. It consists of 3 simple clinical criteria:

  • a breathing rate of 22 bpm or more,
  • Clouding of consciousness and
  • a systolic blood pressure of 100 mm Hg or less.

If two or more of these criteria are met, the likelihood of sepsis with poor clinical outcome is increased. If the criterion is met, the full SOFA score should be determined.
