The uterus didelphys (from ancient Greek δίς, δί dis, di "two" and δελφύς delphys "womb") is a congenital uterine malformation with a double system of the uterus and the vagina .

Synonyms are: bicornuate uterus, bicervical; Uterus duplex, uterus duplex bicollis

pathology

It is a complete duplication of the uterus including a double cervix and often a duplication of the vagina due to a disruption in embryonic development in which the lower parts of the Müller ducts have not merged .

distribution

The uterus didelphys accounts for about 8% of all anomalies in Müller's duct. The prevalence is around 0.5%.

Clinical manifestations

Clinical criteria are:

  • Double arrangement of the corpus and cervix uteri
  • mostly symptom-free, occasionally dyspareunia due to the vaginal septum (in 75%)
  • increased risk of positional anomalies in the unborn child and premature births during pregnancy

There is an association with renal agenesis and transverse vaginal septa with possible obstruction of a horn and unilateral hydrocolpos or hematocolpos .

diagnosis

The diagnosis is usually made by sonography .

Differential diagnosis

Other uterine malformations such as uterus bicornis and uterus septus must be differentiated .

In the context of syndromes

This malformation can also occur in the OHVIRA syndrome (uterus didelphys with obstructed hemivagina and ipsilateral renal agenesis).
