Grew up in Minden / Westphalia , where Winkelmüller graduated from the state Besselgymnasium in 1957. From 1957 to 1962 he studied at the Universities of Göttingen and Munich human medicine and in 1963 in Göttingen with the dissertation radiation fibrosis of the lung Dr. med. PhD . After two years as a medical assistant and license to practice medicine, he worked as a scientific assistant in the Pathological Institute of the University of Göttingen.

In 1966 Winkelmüller began specialist neurosurgical training in the Evangelical Hospital in Oldenburg. In 1969 he was a surgeon at the Bad Zwischenahn Bundeswehr Hospital. From 1969 to 1971 he carried out research on the functional significance of extrapyramidal structures as a scholarship holder at the Max Planck Institute in Frankfurt am Main with stimulus physiological and fiber anatomical studies. He also assisted in stereotactic operations in the Neurosurgical University Clinic Homburg / Saar.

In 1971 Winkelmüller moved to the Neurosurgical Clinic at the Hannover Medical School, where he obtained his specialist certification in 1972. In 1974 Winkelmüller obtained the Venia Legendi with the habilitation thesis The neuronal and functional relationships between the substantia nigra and the striatum . In 1974 he was appointed senior physician in the neurosurgical clinic of the Hannover Medical School, where he was appointed adjunct professor in 1977 and executive senior physician in 1978. In 1972 he introduced spinal cord stimulation in Germany to treat chronic pain.

In 1975 he used cerebellar stimulation to reduce spasticity in early childhood brain damage. He and his team developed experimentally and clinically methods for the quantification of motor disorders in spasticity and rigidity . His operational activities covered the entire spectrum of neurosurgery including pediatric neurosurgery. In addition to lectures in trauma surgery, neurology and endocrinology , Winkelmüller set up an interdisciplinary lecture cycle on the diagnosis and treatment of chronic pain. Together with neuropediatricians, orthopedists and urologists, he founded a working group for the follow-up care of operated spina bifida children.

From 1983 to 1992 Winkelmüller worked as a senior physician at the clinic and group practice for neurosurgery at the Paracelsus Clinic Osnabrück.
