Arkadiusz Protasiuk's tomb in February 2011

On April 10, 2010 Arkadiusz Protasiuk flew as a flight captain together with Robert Grzywna as the second pilot a Polish delegation led by President Lech Kaczyński , who wanted to visit the memorial in Russia on the occasion of the 70th anniversary of the Katyn massacre . During the approach to the destination Smolensk-Nord , there was thick fog and the plane crashed . Protasiuk was killed along with his crew and passengers - for the most part high-ranking representatives from Poland. The official final report of the international Russian Accident Investigation Commission gives Protasiuk the main blame for the disaster, since as the pilot in charge he was responsible for decisive flying errors during the approach. As an explanation of the errors, it is noted that he had to act in a situation of high psychological pressure, which was caused by unauthorized persons in the cockpit during the approach. It is also mentioned that Protasiuk was involved as copilot on a previous flight in which the pilot in command disregarded the president's direct instruction to land at an unsafe airfield.

Protasiuk was married and had two children. During his lifetime he was awarded the bronze medal for services to the defense of the country (Brązowy Medal Za zasługi dla obronności kraju) . Posthumously Protasiuk was promoted to major on April 15, 2010 and was awarded the Knight's Cross of the Order of Polonia Restituta (Krzyż Kawalerski Orderu Odrodzenia Polski) on April 16, 2010 .
