In a major North American city , a scary serial killer , known by the mass media as "The Trasher", is causing panic among the population. By chance, the police succeed in catching the killer in a sewer and arresting him after a short chase. However, he is shot and admitted to the local Crestmore Mental Clinic for first aid, which is under the direction of Dr. Conrad Fallon is standing. Until the FBI arrives and transfers him, the young doctor Dr. Maggie Belham prepare a psychological report on the perpetrator. However, he is sternly silent, can get rid of his chains in an unobserved moment and Dr. Kill Fallon. Dr. Belham can just escape through a laundry chute. Three policemen and two orderlies who arrive shortly afterwards at the scene of the incident are lured into a trap by the killer and killed. Swaggert, the facility's head of security, locks all exits and goes to look for him. However, like another police officer, he is also murdered. Meanwhile, Dr. Belham and Quitz, the caretaker who joined her, are involved in a murderous game of cat and mouse in the catacombs of the clinic, whereby Quitz is killed. The "Trasher" finally escapes through the sewers in the basement, but not without Dr. Tell Belham that he will kill her daughter. The FBI, who has arrived in the meantime, blows up the gates to the institution, but is switched off by a booby trap prepared by the killer. Dr. Belham then takes on the chase through the canals on his own, finds her daughter Sarah locked in a cage and is attacked by the killer. Nevertheless, she manages to free Sarah and kill the "Trasher" with one of his self-made bombs.
