The game form is used to write down a game of chess in order to be able to replay it. The game form usually contains preprinted fields for the names of the two chess players, the date, the venue, the tournament name, the round number and the notation of the game. According to the rules of the World Chess Federation FIDE , it is mandatory for every player to fill out a game form during the game.
