René Fontaine

René Fontaine (born June 5, 1899 in Bischtroff-sur-Sarre , † November 23, 1979 in Strasbourg ) was a French surgeon , he is known for his classification of peripheral arterial occlusive disease (PAD) .

Life

René Fontaine studied medicine in Strasbourg. He was a student of the surgeon René Leriche , whom he followed in 1941 to the chair of clinical surgery at the University of Strasbourg .

Arterial Disease

The classification of the stages of arterial occlusive disease , which is still used in Europe today, comes from René Fontaine .

The 4 stages according to Fontaine are defined as follows:

  • AVK stage I: no complaints
  • AVK stage II: discomfort when walking
  • AVK stage III: complaints at rest
  • AVK stage IIII: tissue death

AVK stage I:

Patients at this stage of PAD are often symptom-free because arteriosclerotic stenoses develop very slowly. This gives the organism time to compensate for the threat of insufficient blood flow ( ischemia ) by forming collaterals . Sometimes, however, at this stage no pulse can be felt and the pelvic or femoral artery is blocked.

AVK stage II:

With physical exertion, discomfort occurs in the corresponding limbs when the collateral network is already inadequate. Typical pain in the lower leg when walking or running, which affect the proceeding, to limp led ( intermittent claudication ), or even to stand still ( "forces claudication ").

AVK stage III:

If the PAD worsens , the pain occurs not only when exercising, but also when the patient is resting.

AVK stage IV:

In the last stage, the most severe form of PAD , the blood flow is so low that the body's own tissue begins to die. The ischemia leads to tissue necrosis, which usually occurs in the toes, as this area is supplied in humans least with oxygen. The necrosis manifests itself as an ulcer or gangrene .
