Vulvar dystrophy

The term vulvar dystrophy (composed of vulva , the Greek dys- (δυσ-) for "miss-" or "un-" and the Greek troph- (τροφή) for "nourish" or "food") is used as a collective name for a number of chronic skin diseases ( dermatoses ) in the area of ​​the vulva. This is for changes in the transitional epithelium with thickened skin or skin shrinkage with largely unknown causes. Most forms of vulvar dystrophy occur after the onset of menopause (climacteric).

Today, the term is considered out of date and has been replaced by specific disease names such as lichen sclerosus or vulvar intraepithelial neoplasia (VIN).

There are three main types of vulvar dystrophy:

  1. hyperplastic dystrophy, also known as leukoplakia of the vulva. This form of precancer is characterized by severe keratinization of the mucous membranes.
  2. the atrophic dystrophy, also known as kraurosis of the vulva. In this case, the vulva shrinks.
  3. mixed dysplasia with numerous atypical cells. This form can lead to malignant changes in the vulvar epithelia and vulvar cancer of the labia majora, less often of the labia minora and the area of ​​the clitoris.

Diagnosis and therapy

The diagnosis is made in the form of a histological and clinical differential diagnosis of malformations ( genital dysplasia ), especially vulvar intraepithelial neoplasia (VIN), and virally induced erythroplasia . Proof of atypical changes is necessary for classification as precancerous .

Depending on the inspection results are used to treat ointments and gels which, sex hormones ( estrogen ) or corticosteroids contained or used systemic estrogen doses. In the case of advanced dysplasia and suspicion of malignant tumors, the identified foci are surgically removed ( vulvectomy ).
