Pull ointment

Zugsalbe or drawing ointment are ointments which are sold to stimulate blood circulation, anti-inflammatory, analgesic, talgflussvermindernde, absorption and phagozytosefördernde own properties. They are mostly made from oil shale , more rarely also on a vegetable basis. Pull ointment can be used for chronic inflammatory skin diseases ( contact eczema , folliculitis , boils , acne , abscesses under the skin, phlegmon and panaritium ) as well as smaller splinters. In human medicine, pull ointment with ammonium bituminosulfonate is approved as a medicinal substance. This active ingredient is also used in the veterinary field.

Mechanism of action

Due to the content of sulfonated shale oil ( ammonium bituminosulfonate , ichthammolum), draft ointment has an antibacterial, anti-inflammatory and antipruritic effect. In addition, the bituminosulfonates soften the skin and increase blood flow. In the case of purulent skin processes, in the advanced stage of inflammation it accelerates the melting down of the focus of the disease and the breakthrough of the pus to the outside ("pull", "pull").

For the maturation of the abscess or boil, a brief application of heat e.g. B. by infrared radiation. It is believed that pulling ointment promotes spontaneous perforation . On the other hand, it is stated that pulling ointment softens the skin over the abscess and thereby matures the abscess, but that the abscess must then be surgically opened by the doctor.

effectiveness

Few studies are currently (as of 2019) carried out on the effectiveness of ammonium bituminosulfonate ointment.

  • One study demonstrated the superiority of ammonium bituminosulfonate ointment in the treatment of venous ulcers , but only for a short time frame.
  • Due to possible risks, the application with reference to § 5 of the Medicines Act is viewed as very critical.
  • Studies on the effectiveness of abscesses have not been carried out in the last 20 years (as of 2019).
  • In the standard work of Pichlmayr's Surgical Therapy , the use of anal abscesses is rated as outdated.
  • Studies on the effectiveness of otitis externa are mainly comparative studies with comparisons with other preparations, with partly contradicting results.

Side effects can occur if there is an allergy to the components. If you are pregnant or breastfeeding, you must consult your doctor beforehand.

Ichthyol Society

In Germany, the use of ointments containing ammonium bituminosulfonate is propagated by the Ichthyol Society. The company is owned by Cordes Pharma GmbH, which manufactures and markets ammonium bituminosulfonate under the Ichthyol trademark .

Medical regulations of veterinary medicine

Ammonium and sodium bituminosulphonate are approved for external use in all food-producing animals (including milk-producing animals) and are listed in Annex II of Regulation (EEC) No. 2377/90 on maximum levels for veterinary drug residues in food . However, a small part of the ingredients is absorbed and can lead to changes in the taste of the meat or milk.

Traction plaster

Commercially provided also with Zugsalbe patches available. Pull plasters (pull plasters, attraktiva) were already in use in the Middle Ages (zuc-phlaster or zuc for short ) to remove toxins, exudates and pus from ulcers and open wounds.
