On December 23, 1973, a Tupolev Tu-124 crashed on the inner-Soviet scheduled flight Aeroflot flight 5398 from Lviv via Kiev to Kuibyshev , killing all 17 occupants.

Plane and crew

The plane was a 10-year-old Tupolev Tu-124W with the air vehicle registration number , the 2-45044 СССР turbofan engines of the type Solowjow D-20 was equipped P.

The crew consisted of a flight captain , a first officer , a flight engineer , a navigator , a flight attendant and a police officer ( MWD ).

course

The Tu-124 took off from Lviv Airport at 10:04:49 p.m. and headed southeast. The responsible air traffic controller in Lviv instructed the pilots to climb at 1,800 m in the direction of the Zolochiv NDB and to make radio contact at 1,500 m. The pilots reported to the air traffic controller 2 min 15 s after take-off and reported that the left engine was burning and that they had to turn around and make an emergency landing as a result. Because the air traffic controller did not understand this, the pilots had to repeat this twice. After that there was no further radio contact.

The partially burned wreck was finally found 18.3 km east of Lviv Airport, near Wynnyky, on impassable terrain.

root cause

The debris field was 725 m long and 125 m wide. Small, burned aircraft parts were found 3 to 5 km away from the main wreck.

It turned out that one of the blades of the first compressor disk in the left engine had come loose, causing violent vibrations that led to the rupture of one or more lines containing flammable liquids (fuel, hydraulics , oil). As a result, a fire broke out 1 min 55 s after take-off, which ultimately ate its way through the outer skin and the cabin floor on the left side . The control rods also showed traces of heat exposure. Since the fire did not allow a return to the airport, the pilots probably decided to land in the area. The aircraft finally hit the ground flat at 10:08:04 p.m. at a speed of 500 km / h .

The cause of the bucket loosening was the failure of the retention pins , which had sub-par load capacity due to improper heat treatment during manufacture.
