After the death of Mathew Makil (1851-1914), the first Vicar Apostolic of Kottayam, Chulaparambil succeeded him in office. He had served as his secretary before. On July 16, 1914, Pope Pius X appointed him Vicar Apostolic of Kottayam and Titular Bishop of Busiris . The episcopal ordination donated him the Apostolic Delegate of East India, Archbishop Ladislaus Zaleski (1852-1925), on November 1st of that year.

On December 21, 1923, Pope Pius XI called the diocese of Kottayam, as the diocese of all Catholic Knananites ; Alexander Chulaparambil became the first bishop.

Bishop Chulaparambil was a great devotee of the Sacred Heart of Jesus and valued the spirituality of St. Francis de Sales . In 1921 he founded the Salesian Order of Men of the Oblates of the Sacred Heart of Jesus (OSH) , which still exists today. In 1932 he visited Europe and also went to Konnersreuth to get to know the stigmatized Therese Neumann personally. The pastor led and consolidated the Diocese of Kottayam with great care and excellent commitment until his death in 1951. As Vicar Apostolic and Bishop he was the head of the Catholic Knananites for 35 years.

The diocese of Kottayam financed a larger colony for Knananite Catholics in Madampam near Kannur in 1943 , where around 100 such families settled. The place was named "Alexnagar", derived from the first name Bishop Chulaparambils.
