The brilliant head physician of a state mental hospital Dr. Colin Ramzi secretly abuses the patients in his clinic for horrific experiments. The insane and death-obsessed surgeon takes the brains of his living subjects in an underground laboratory , since he sees in it the cause of all human evil. When his older colleague Dr. Swan urges him to reason that he fears for his professional career and a little later catches Ramzi taking an illegal brain harvest, he kills him in self-defense and walled him up together with the trial victims.

20 years later, a young woman is found wandering on the street and admitted to the psychiatric clinic without any memory of her past. Swan is headed. In the absence of her real name, the young woman simply calls herself Jane Doe . The doctors diagnosed her with amnesia , but she defends herself against this finding and insists that part of her brain had been surgically removed in the past, depriving her of all memories. Shortly after they were admitted, a strong earthquake occurred, which unfortunately hit the sealed underground grave of Dr. Ramzi opens, so that he wakes up as the undead together with his victims, who have mutated into zombies . Immediately after his liberation, the revenant starts looking for new victims in order to resume his bizarre attempts and at the same time take revenge on Swan and his entourage. In this way, he gradually kills both patients and employees of the institution, initially unnoticed by the clinic staff.

Head physician Dr. Swan to initiate hypnotherapy with Jane in order to reach her subconscious. The amnesia patient, plagued by nightmares, who, under the traumatic visions of the blood-smeared surgeon Dr. Ramzi suffers and unconsciously confronts Swan with his own past, whereupon he anxiously breaks off the hypnosis. Jane suspects something ominous, but at first cannot explain the supernatural events. Since no one wants to believe her, she turns to the explosives expert Chris to examine a disused building section at night. In the asylum's clock tower, Jane meets the undead Dr. Ramzi and his zombie character.

The madman then attacks the adjoining clinic together with his living dead and kills a large number of defenseless patients. The psychiatric clinic falls victim to the host of creatures within a very short time. By chance, Jane and Chris discover that the bloodthirsty creatures are extremely susceptible to holy water and can thus be fought effectively. At the end of the film - Dr. Swan is already dead - the "contaminated" area is flooded by blowing up a consecrated water container; all creatures are destroyed. Jane realizes her true identity in the midst of the chaotic situation - the maniac was her father - and apparently takes over his bloody work.
