Its entire course through the sparsely populated south of South Lanarkshire follows primarily a northeasterly direction. The Glespin Burn, one of the largest tributaries, flows into the hamlet of Glespin . The Douglas Water touches the village of Douglas and flows into the Clyde after a total of 32 km near New Lanark above the Falls of Clyde .

From Glenbuck Loch, the A70 ( Edinburgh - Ayr ) roughly follows the course of Douglas Waters and crosses the river once. Furthermore, the M74 runs over the river.
