The human herpes virus 7 (HHV-7) is a herpesvirus which the subfamily of the beta herpes viruses belongs.
meaning
Infection with HHV-7 usually occurs in childhood as an asymptomatic infection or as an exanthema subitum . This is followed by a lifelong latency of the virus in the body. In adolescence at the latest, an infection rate of around 95% is assumed. Reactivations are particularly problematic in immunosuppressed patients, for example after organ transplants. In addition, reactivations of HHV-7, as well as reactivations of the related human herpesvirus 6 (HHV-6) , are discussed as triggers of the skin disease pityriasis rosea .
Further associations of HHV-7 were found in various other clinical pictures, although the clinical significance of the infection is often unclear. In children and immunocompromised adults with diseases of the central nervous system , HHV-7 can in some cases be detected in the cerebrospinal fluid . In exacerbations of interstitial lung diseases , HHV-7 can be detected in nasal secretions, sputum or in the bronchoalveolar lavage . There is also evidence that HHV-7 is a contributory factor in myocarditis in children.
Diagnosis
The diagnosis can be carried out serologically or by means of PCR . Serological diagnostics using ELISA or immunofluorescence are considered less specific, as cross-reactivity with HHV-6 and CMV can occur here. The PCR detection, however, is specific and highly sensitive.
therapy
Treatment of the infection with HHV-7 is usually not necessary and is only carried out in immunosuppressed patients. The antiviral agent ganciclovir appears to be able to lower the viral load of HHV-7. The active ingredients foscarnet and cidofovir are also used for therapy.
