The Little Sandy River is a left tributary of the Ohio River in the US state of Kentucky .

The Little Sandy River rises southwest of Sandy Hook in Elliott County . It flows through the counties Elliott, Carter and Greenup in a north-northeast direction and flows after 137.4 km at Greenup in the Ohio River. Kentucky Route 7 runs along its upper reaches . The river passes the place Sandy River and the small town Sandy Hook. The Little Sandy River then flows through the Grayson Lake reservoir . From Grayson , Kentucky Route 1 runs along the river for a while. Above Argillite , Claylick Creek , the outlet of Greenbo Lake , flows into the Little Sandy River from the left. A few kilometers downstream, the East Fork Little Sandy River , the largest tributary, flows into the river from the right. The Little Sandy River is accompanied by Kentucky Route 2 for the last few kilometers to its mouth . The river has numerous meanders .
