The Münsteraner Kreis is an interdisciplinary association of 17 personalities from different disciplines with expertise in the field of complementary and alternative medicine (KAM) . He advocates a consistent evidence base within the public health system and the independent medical professions. The circle's criticism is particularly directed against homeopathy and the current training as a naturopath . He has published a memorandum on each of these two topics .

In addition to medical considerations, the group also wants to include ethical, scientific theory, psychological and legal aspects in its considerations. With his work he intends to contribute to the public discourse about the KAM and its role in the health care system. The work of the circle is not financially supported by third parties.

history

The merger to form the Münsteraner Kreis took place in June 2016 on the initiative of medical ethicist Bettina Schöne-Seifert from the Westphalian Wilhelms University of Münster. He first appeared publicly in July 2017 with the so-called Münsteraner Memorandum Heilpraktiker, a theses paper on the problem of the health practitioner status in Germany. The establishment of the Münsteraner Kreis received wide media coverage in connection with the publication of the memorandum.

The initiator stated that the aim was to “sound out how a health system based on solidarity should deal responsibly and fairly with the clash between dangerous pseudoscience and self-determination. To put it clearly: We did not want to accept the current madness any longer. ”The Munsteraner Kreis sees irreconcilable parallel worlds in the existence of academic medicine on the one hand and the sphere of complementary and alternative medicine (KAM) on the other. While academic medicine strives for evidence-based and justified progress, alternative practitioners are anchored in the predominantly unscientific world of thought of complementary and alternative medicine (KAM). For many years there have been repeated, sometimes intensive, discussions on the subject of complementary and alternative medicine. Numerous clinical studies have been carried out on the hundreds of procedures, but their quality is often very poor. Convincing evidence of effectiveness would mostly be lacking. In addition, the traditional disease concepts and interventions of the KAM often contradict fundamental scientific findings.

The activities of the Münsteraner Kreis aim to provide impetus in the context of public discourse and with regard to health policy under the aspect of quality assurance and thus patient protection. The district understands the “Münsteraner Memorandum Heilpraktiker” as a first concrete activity in the overall framework.

Münster memorandum alternative practitioners

As the first work result of the Münsteraner Kreis, the "Münsteraner Memorandum Heilpraktiker" was published on August 21, 2017 as a full text in Deutsches Ärzteblatt and on the initiative's website.

The Memorandum locates the application of methods of complementary and alternative medicine without scientific proof of benefit mainly in the field of alternative practitioners. The Münster district does not consider the low admission requirements and the lack of training requirements for "practicing medicine without appointment" (the legal designation of alternative practitioner activity), in particular the minimal technical requirements compared to a scientific medical degree, to be justifiable. Based on these premises, the memorandum sees potential for harm to patients when such methods are used by both alternative practitioners and doctors. Against this background, the memorandum deals with the question of how a regulatory framework could be created within the framework of weighing up patient autonomy and therapy freedom, but also under the aspect of fairness within solidarity health insurance.

As a solution, the memorandum suggests the abolition of the previous alternative practitioner construct or the creation of a "specialist alternative practitioner" with a clearly defined scope of activity and training at the technical college level. The reactions in the media, in specialist publications and from the organized medical profession and also from the field of alternative practitioners and their interest groups were varied.

The memorandum will also be discussed in the context of the proposals of the federal-state draft under the leadership of the Ministry of Health for a reform of the medical practitioner examination.

The German Network for Evidence-Based Medicine (DNEbM) took up the content of the Münster Memorandum in a statement dated October 24, 2017. In it, the DNEbM declares its clear preference for the abolition of the state permit to practice medicine as a non-medical practitioner. With consistent evidence-based evidence in all health professions, it does not recognize a sufficient basis for a qualifying course to become a "specialist medical practitioner", as the Münster Memorandum considers an alternative to be conceivable, nor does it provide a solution to the qualification problem. The DNEbM also sees little point in the professional profile of specialist naturopaths because, in its view, the possibilities of transferring medicine in model projects according to the Nursing Development Act of 2008 and the Medical Transfer Directive of the Joint Federal Committee (according to Section 63, Paragraph 3c of the Social Code Book V) exist to nursing professionals and the potential this offers transferring healing activities to nursing is by no means exhausted.

Münster Memorandum Homeopathy

In February 2018, the Münsteraner Kreis asked the 121st German Medical Association to delete without replacement the additional designation "Homeopathy", which had previously been awarded to doctors by the medical associations for advanced training in homeopathy .

Since homeopathy does not rely on pharmacologically and physiologically plausible active ingredients, but on immaterial, spirit-like active forces, it must be classified as esoteric. In addition, hundreds of clinical studies and numerous reviews for homeopathy would have resulted in negative assessments from national and international bodies.

The Münster circle stated that the homeopathy number was often used in everyday care as a substitute for the lack of billing ability of medical communication, and warned against apparently "repairing" the problems of scientific medicine in an unscientific parallel world. Rather, these would have to be resolved within scientific medicine. Even the administration of homeopathic medicines as covert placebos should be rejected because it violates the patient's right to honest information and at the same time adele and maintain a 'parallel medicine'.

Even in the run-up to the 121st German Medical Congress, the homeopathy memorandum met with concerns. The President of the Westphalia-Lippe Medical Association demanded that the health insurance companies should no longer reimburse the costs of homeopathy as part of statutory benefits. However, he refused to abolish the additional designation, since otherwise homeopathy would end up in the hands of non-medical professional groups. On May 11, 2018, the Doctors' Day confirmed the additional designation "Homeopathy".
