The tarsal tunnel syndrome is a rare nerve compression syndrome of the tibial nerve at the ankle in the tarsal tunnel .

root cause

The nerve compression in the flexor retinaculum on the inside of the ankle joint can be the result of a broken bone, a sprain, or it can occur idiopathically for no apparent reason.

Clinical manifestations

Clinical criteria are:

Hypesthesia , paresthesia , sometimes dysesthesia in the foot area, development of paresis of the foot muscles and pain on pressure over the distal course of the tibial nerve.

diagnosis

Detection and classification criteria for electromyography were described by PE Kaplan . Apparently the lateral branch of the nerve is affected earlier than the medial one. The diagnosis is based on the clinical-neurological and neurophysiological findings.

An imaging examination is required to clarify the cause or if nerve tumors or ganglia in the tarsal tunnel are suspected .

therapy

A surgical procedure is carried out at therapy-resistant dysesthesia or neurological deficits by cleavage of the flexor retinaculum in the tarsal tunnel and extensive exposure of the nerve. The success rate is considered very good.

history

As early as 1932, L. Pollock and L. Davis published a case report on post-traumatic compression of the tibial nerve; the symptoms of the tarsal tunnel syndrome were described in 1960 by HP Kopell and WA Thompson . The term was coined in 1962 by Charles Keck and SJ Lam .
