Thomas Bschleipfer

Thomas Bschleipfer (born January 26, 1972 in Augsburg ) is a German doctor and philosopher. He is a specialist in urology , professor at the Justus Liebig University in Giessen and chief physician at the clinic for urology, andrology and pediatric urology at the Klinikum Weiden / Kliniken Nordoberpfalz AG. Bschleipfer is an expert in the areas of functional urology / bladder dysfunction , urogynecology, interstitial cystitis / bladder pain syndrome (IC / BPS) and benign prostate syndrome (BPS). Scientifically, Bschleipfer dealt in particular with the clinical picture of the overactive bladder (OAB).

In philosophy, Bschleipfer specialized in the field of moral philosophy and dealt intensively with allocation problems . He wrote the first ethics of German crisis and emergency medicine.

Life

After graduating from high school near St. Stephan in Augsburg, Bschleipfer studied human medicine at the University of Ulm (1992–1999) and received his doctorate in 2002 on the subject of "Experimental blunt kidney trauma - biomechanics, trauma behavior and diagnostic imaging". med. "(summa cum laude).

In 1996, Bschleipfer completed his accompanying humanities studies with a focus on philosophy at the Humboldt Study Center of Ulm University on the subject of "virtue ethics - the problem of so-called secondary virtues ". From 1999 to 2007 he was enrolled as an external doctoral student at the BTU Cottbus and received his doctorate at the chair for philosophy of technology with Klaus Kornwachs in 2007 as “Dr. phil. ”(magna cum laude). The topic of the thesis was "Ethics of a Crisis Medicine - Critical Analysis of Area-Specific Dilemmas: Resource Allocation, Instrumentalization and Double Loyalty".

After working in the surgery, urology and anesthesia departments at the Bundeswehr Hospital in Ulm (1999–2001), Bschleipfer was a medical officer and aviator of the Airborne Warning and Control System (AWACS) / E-3A Association of NATO in Geilenkirchen (2001–2004). From 2004 to 2014 he worked in the clinic and polyclinic for urology, pediatric urology and andrology at the Justus Liebig University in Giessen ( Wolfgang Weidner ). Bschleipfer was recognized as a specialist in urology in 2008. In 2009, the European Board of Urology (EBU) awarded him the title “Fellow of the European Board of Urology” (FEBU). In 2010, Bschleipfer was appointed senior physician at the clinic and was head of the urodynamics and neurourology section. Bschleipfer completed his habilitation in 2014 on the subject of "The overactive bladder - cholinergic systems of urinary bladder sensors ". He received the Venia legendi and the academic title “Dr. med. habil. ”and“ Privatdozent ”. In 2016 he was appointed professor at the Justus Liebig University in Giessen. Since 2014, Bschleipfer has been chief physician at the Clinic for Urology, Andrology and Pediatric Urology at the Klinikum Weiden / Kliniken Nordoberpfalz AG. It has the following additional medical designations: ' Naturopathic Treatment ' (2004), 'Emergency Medicine' (2006), 'Medicinal Tumor Therapy' (2010) and 'Andrology' (2010).

Scientific contribution

Bschleipfer began his scientific career in the field of urological traumatology . As part of his medical doctorate, he dealt with the biomechanics and trauma behavior of kidneys after blunt injuries. His work was able to demonstrate a gradual addition of different types of lesions at critical energy and decompensation limits with progressively increasing stress . This showed a significantly increased trauma susceptibility to organs with restricted drainage. A direct comparison of macroscopy and contrast agent computed tomography showed a very high level of diagnostic accuracy for the sectional image method. In the following, Bschleipfer dealt with functional urology, especially with the clinical picture of the overactive bladder (OAB). Bschleipfer's research focus was on the cholinergic system of urinary bladder sensors. Different muscarinic and nicotinic receptor subtypes and subunits could be identified on the afferents of the urinary bladder. Changes could also be shown which arise after obstruction of the bladder outlet, such as an enlarged prostate in the aging man. In addition, Bschleipfer's research was able to show a non-neuronal cholinergic system within the urothelium (bladder mucous membrane), which also expresses cholinergic receptors, does not differ between men and women and shows no significant changes in the presence of a bladder outlet obstruction. Further work by Bschleipfer was able to demonstrate a significant influence of systemic arteriosclerosis on the development of detrusor overactivity (DHA) and thus on the clinical picture of the overactive bladder (OAB). Together with Wolfgang Kummer , Director of the Institute for Anatomy and Cell Biology at the Justus Liebig University in Giessen, Bschleipfer worked on the first description of the urethral brush cell, a taste cell in the urethra that could be detected not only in humans but also in numerous mammals . This chemosensory “guardian cell” is able to “taste” bitter substances and special molecules (quorum-sensing molecules, QSM) from bacteria (penetrating the body). This triggers a reflex contraction of the bladder, which leads to the elimination of potential pathogens.

To date, Bschleipfer has published more than 45 PubMed-listed publications, held more than 250 national and international lectures, organized several congresses and seminars and wrote numerous (teaching) book contributions. As a member and secretary of the Benign Prostatic Syndrome Working Group (AK BPS) of the German Society for Urology (DGU), he was significantly involved in the development of the S2e guideline "Therapy of Benign Prostatic Syndrome (BPS)" and is currently working on the guideline "Diagnostics of Benign Prostatic Syndrome (BPS) ". He is a member of the guideline commission of the working group for urogynecology and plastic pelvic floor reconstruction AGUB eV of the German Society for Gynecology and Obstetrics eV (DGGG) for the development of the guideline "Diagnostics and Therapy of Overactive Bladder in Women". He is also the guideline coordinator of the guideline "Diagnostics and Therapy of Interstitial Cystitis (IC / BPS)".
