Supportive periodontal therapy

Under a supportive periodontal therapy ( UPT ; Engl. : SPT - Supportive Periodontal Therapy; also Parodontalnachsorge , maintenance therapy as well - not quite correct - Recall) is understood in the dentistry a systematic follow-up program that integrates seamlessly with the dental treatment of periodontitis should be followed. Since teeth that have previously been periodontally diseased and successfully treated can have a lifelong recurrence (a flare-up of periodontitis), the UPT should detect this as early as possible, delay it or even prevent it completely.

Content

The condition of the teeth and especially the interdental spaces is shown and checked by staining the bacterial plaque . The patient is informed and, if necessary, remotivated to improve their oral care. Then all hard and soft plaque and deposits are removed. The tooth surfaces are polished. At regular intervals (around 6 to 12 months), the depth of the gingival pocket and possible bleeding tendency from the pocket are measured at several points on each tooth with special probes and recorded as a PSI code . These and other findings are summarized in the periodontal status, which enables a precise description of the condition of the tooth support system (periodontium).

Supportive post-implant therapy

Very similar mechanisms that lead to periodontitis in the natural tooth are also observed in dental implants that lead to peri-implantitis . As with supportive periodontal therapy, supportive post-implantological therapy (UIT) is recommended.

execution

It is carried out by means of professional tooth cleaning (PZR). It is understood as a comprehensive mechanical cleaning of the teeth, which eliminates the deficits in daily oral hygiene. The UPT is in Germany by the dentist or by appropriately formed continuing dental assistants ( Dental prophylaxis assistants , dental Medical Assistant , Dental Hygienists ) provided. For all patients, the UPT is usually carried out every 3 to 6 months or the frequency of implementation is determined individually depending on the risk.

Patient acceptance

According to scientific studies, patient acceptance (compliance) in UPT is only around 30% after 3–5 years worldwide. There is hardly any data on patient compliance in the UIT. A study by the University of Freiburg found acceptance rates of around 90% in the first three years.

forecast

In patients with good UPT compliance, the prognosis is relatively good even for teeth that are already more severely diseased, and these can often be maintained for years.

Billing

The UPT is not a statutory health insurance service. The costs for a PZR amount to around 35 to 150 euros, depending on the number of teeth, effort and design. With the entry into force of the amendment to the fee schedule for dentists (GOZ) on January 1, 2012, according to fee item 1040, with average expenditure (2.3 times the factor), 3.62 euros per tooth will be charged.
